Whom is ceri Personalized patient program for?
It is for everyone who seek answers for their cancer-related issues - cancer patients, their family members, or for everyone who have questions related to cancer, cancer prevention, healthy lifestyle and diet, and more.
Is CERI Personalized Patient program free?
Application for our CERI Personalized Patient Program has a minimal fee. All inquries are subject to an initial fee of $45. After evaluation, if full patient service and research is needed, we ask for an additional $30. These fees are subject to change.
I have low or no income. Do you waive the fees or make discounts?
In this case, we will require documentation of proof of low income or no income. If you will not be able to pay the program fee, we may help you to find a sponsor to cover your program fee. This may delay the timing of the program support.
I am a cambridge, Massachusetts resident. Is CERI Personalized Patient Program free for me?
Yes. Thanks to Cambridge Community Foundation, ALL Cambridge, MA resident's program fee are covered.
What will happen after I submit my CERI Personalized Patient Program inquiry?
You will contacted - via the e-mail you have provided - by the CERI Team. The e-mail will provide you with the step-by-step instructions on the patient intake process and will send you a separate invoice for the program fee. If you do not see the e-mail in your inbox, please check your spam folder and make sure you mark it as 'not spam'.
How quick will I receive a response after I submitted my inquiry?
We usually respond within 24-48 hours of inquiry submission. For CERI Personalized Patient Program inquiries, if requested documents are incomplete, this will cause major delays. The respond time is also subject to change based on the volume of inquiries, staff, and urgency of the cases as determined by the CERI Team after evaluation.
